Shouldn't line 1 read
angle = 2 * Math.acos(q1.w);

(Since -1 <= w <= 1, 2w can be >1 or <-1. acos(x) is undefined if this is the case)

Yes, I think your right. I have corrected the page and also added a derivation as a check.
